A Systematic Review: Non-entry of eligible patients into surgical randomised control trials (RCTs) is the most significant barrier to their success of & the most serious threat to the validity of their findings.There was apparent discrepancy between its real & perceived reasons.The most common reasons for non-entry were patients preference, dislike of the idea of randomisation, difficulties with consent & protocols & the clinicians loss of motivation due to lack of recognition.A Prospective Study: Reasons for non-entry of eligible patients in the Australasian Laparoscopic Colon Cancer Study (ALCCaS) were prospectively recorded for a period of 6 months.Out of 113 eligible patients, 62(55%) were not entered.The most commonly recorded reasons were having a preference by the patient or the surgeon for one form of surgery (73%) & lack of time, staffing or equipment.Recruitment correlated positively with the availability of a data manager & negatively with an increased caseload.
