Human immunodeficiency virus associated neuropathy is the most common neurological disorder during HIV infection and HIV represents one of the main risk factors for development of a neuropathy worldwide.Distal symmetric polyneuropathy (HIV-DSP) is the most common manifestation in HIV infected patients and shows unpleasant sensory symptoms in nearly 50 % of the patients.This pain arises as a direct consequence of HIV infection of the somatosensory system.Symptoms include e.g.continuous pain, allodynia and hyperalgesia.Besides HIV infection itself medication may take part in the development of HIV-DSP.Causal treatment of HIV-DSP means application of highly active antiretroviral therapy (HAART).Additive cofactors especially in elder HIV infected patients (>50 years, diabetes, vitamin lack) must be considered.Symptomatic treatment of neuropathic pain includes use of antiepileptic medication, tricyclic antidepressive agents, opioids and local treatment with the high dose capsaicin patch.Small fiber HIVassociated DSP represents an increasing problem to the fact that this form of HIV-associated DSP is not detectable by using neurophysiological routine methods.Adequate treatment of neuropathic pain improves quality of life and patients return to daily functioning.
