目的探讨养阴生津与清热解毒法治疗鼻咽癌放疗口咽黏膜反应的临床疗效。方法选择2005年1月至2012年8月间收治的110例鼻咽癌患者为研究对象,采用信封法将110例患者随机分为治疗组和对照组,每组55例,对照组患者予以常规西医治疗,治疗组患者予以百合固金汤和鼻咽清毒剂治疗,观察两组患者放疗期间的口咽黏膜反应程度、出现放射性口咽黏膜反应时间及不良事件。结果放疗3周后的口咽黏膜反应程度以1、2度为主,放疗7周后的口咽黏膜反应程度以2、3度为主。两组患者放疗3周、7周后的口咽黏膜反应程度差异均有统计学意义(χ2=8.391,9.961;P=0.015,0.017)。治疗组患者出现1度放射性口咽黏膜反应症状的初发时间为(19.23±1.85)d,对照组则为(15.89±2.03)d,差异有统计学意义(t=9.019,P<0.001)。两组患者均未出现恶心、呕吐、胃肠不适等症状出现。结论养阴生津与清热解毒法可延迟鼻咽癌放疗口咽黏膜反应的出现时间,有利于放疗的进行。
Objective To investigate the clinical effects of Yangyin Shengjin and Qingrejiedu treatment of nasopharyngeal carcinoma radiation oropharyngeal mucosa reaction. Methods One hundred and ten patients with nasopharyngeal carcinoma admitted from January 2005 to August 2012 were enrolled in this study. One hundred and ten patients were randomly divided into treatment group and control group by envelope method, with 55 cases in each group. Patients in control group were treated routinely Western medicine treatment, treatment group patients were treated with Lily Ginkgo Decoction and nasopharyngeal toxicant, the two groups of patients during or after radiation therapy to observe the degree of oropharyngeal mucosa reaction, radioactive oropharyngeal mucosal reaction time and adverse events. Results The degree of oropharyngeal mucosal reaction was mainly at 1 and 2 degrees after 3 weeks of radiotherapy. The degree of oropharyngeal mucosal reaction at 7 weeks after radiotherapy was mainly at 2,3 degrees. There was significant difference in the degree of oropharyngeal mucosal reaction between the two groups after radiotherapy for 3 weeks and 7 weeks (χ2 = 8.391,9.961; P = 0.015,0.017). The onset time of first degree radioactive oropharyngeal mucosal reaction was (19.23 ± 1.85) d in the treatment group and (15.89 ± 2.03) days in the control group, the difference was statistically significant (t = 9.019, P <0.001). Two groups of patients did not appear nausea, vomiting, gastrointestinal discomfort and other symptoms appear. Conclusion Yangyin Shengjin and detoxification method can delay the emergence of nasopharyngeal carcinoma radiation oropharyngeal mucosa reaction time is conducive to the progress of radiotherapy.
