目的:探讨原发性高血压患者踝臂动脉脉搏波传导速度(baPWV)与颈动脉内膜中层厚度(IMT)及中心动脉压(CAP)的关系。方法:收集原发性高血压患者共444例,对所有患者进行IMT、baPWV及CAP的测定。根据baPWV分为异常组(baPWV≥14m/s,208例)和正常组(baPWV<14m/s,236例)。用简单相关分析检测baPWV与CAP及IMT的相关性。采用Logistic回归分析baPWV与CAP及IMT的关系。结果:Pearson相关分析显示,baPWV与年龄、24h平均收缩压(24hSBP)、白昼平均收缩压(dSBP)、夜间平均收缩压(nSBP)、中心动脉收缩压(CASP)、中心动脉舒张压(CADP)、中心动脉脉压(CAPP)、反射波压力(AP)、中心动脉压增强指数(AIx)、75次心率AIx校正指数(AIx75)及IMT相关(P<0.05)。进一步控制年龄、24hSBP、dSBP、nSBP后,CASP、CADP、CAPP、AP、AIx75、IMT仍与baPWV相关。Logistic回归分析显示,CASP、CAPP、IMT与baPWV相关。结论:CASP、CAPP、IMT是baPWV增加的危险因素。
Objective: To investigate the relationship between ankle brachial artery pulse wave velocity (baPWV) and carotid artery intima-media thickness (IMT) and central arterial pressure (CAP) in patients with essential hypertension. Methods: A total of 444 patients with essential hypertension were collected and the IMT, baPWV and CAP were measured in all patients. According to baPWV, they were divided into abnormal group (baPWV≥14m / s, 208 cases) and normal group (baPWV <14m / s, 236 cases). Correlation of baPWV with CAP and IMT was detected by simple correlation analysis. Logistic regression analysis of baPWV with CAP and IMT relationship. Results: Pearson correlation analysis showed that the relationship between baPWV and age, mean systolic blood pressure 24h (24hSBP), mean systolic blood pressure (dSBP), nSBP, CASP, CADP CAPP, AP, AIx, 75 AIx75 and IMT (P <0.05), respectively. CASP, CADP, CAPP, AP, AIx75 and IMT were still associated with baPWV after further control of age, 24hSBP, dSBP and nSBP. Logistic regression analysis showed that CASP, CAPP and IMT were associated with baPWV. Conclusion: CASP, CAPP and IMT are the risk factors of baPWV increase.
