Aim To investigate the diagnostic and prognosis value of high-sensitivity cardiac troponin T (hs-cTnT) for early detection in patients with acute coronary syndrome (ACS).Methods The hs-cTnT, CK-MB, myoglobin (Myo) and troponin T (cTnT) were detected in blood samples in order to discuss the diagnostic value of hs-cTnT in patients with acute myocardial infarction (AMI), or stable angina, or unstable angina and heart failure, and compare hs-cTnT concentration changes of ACS patients over the following time: the admission time, 6 h after admission and the recovery time, α represents the percentage change of concentration detected between the admission time and 6 h after admission, and β represents the percentage change detected between the admission time and the recovery time.Results hs-cTnT in patients with early AMI was significantly higher than that of patients with other diseases and heathly ones;Compared with cTnT, hs-cTnT has statistically significance in AMI and unstable angina.Based on the standard diagnostic strategy of hs-cTnT≥30 ng/L, 47 patients were diagnosed to have AMI.The hs-cTnT sensitivity in diagnosing AMI at the admission time and 6 h after admission (95% confidence interval) were 72.2% (53.1% ~ 84.1%) and 93.8% (83.7% ~ 98.2%) ,respectively.The corresponding specificity (95% confidence interval) were 79.1% (71.9% ~ 86.3%) and 84.3% (78 1% ~89.4%) ,respectively.When α alteration exceeded 30%, diagnosis specificity reached 91.4% (diagnosis sensitivity 63.5% ~86.4%).In follow-up period, 2 patients died from cardiac death.Conclusion Hs-cTnT is a sensitive biomarker for AMI, it has higher sensitivity than cTnT in early diagnosis of patients with AMI, and has the same specificity with cTnT.It may improve ACS symptoms associated specificity and risk assessment by comparing hs-cTnT concentration changes detected between the first detection time and the follow-up ones.
