Introduction : A large intra-atrial thrombus extending from inferior venae cava is very rare and grave clinical occurrence.Free floating intracardiac thrombus is a rare incidental imaging occurrence and requires urgent open heart surgery.We reporting a case of young African American female with pelvic abscess was found to have large intracardiac thrombus, arising from IVC filter.Patient underwent urgent surgical thrombolectomy with cardioplegia.
