目的:观察不同程度羊水胎粪污染新生儿的脐动脉血气p H值与血糖结果关系。方法:2013年5月1日-2014年6月1日产科分娩484例不同程度羊水胎粪污染的新生儿,其中羊水胎粪污染Ⅰ°160例、羊水胎粪污染Ⅱ°135例、羊水胎粪污染Ⅲ°189例。对新生儿进行脐动脉血气p H值及血糖检测结果分析。结果:脐动脉血血气指标p H值及血糖结果差异显著,Ⅰ°、Ⅱ°羊水胎粪污染程度与脐动脉血血气指标p H值及血糖成正比关系,Ⅲ°羊水胎粪污染的新生儿脐动脉血血气指标p H值及血糖成反比关系。结论:不同程度羊水胎粪污染新生儿的脐动脉血气p H值与血糖结果差异显著,且与新生儿窒息缺氧的程度有密切联系,有助于判断胎儿血气异常的性质及其严重程度,对于分析胎儿娩出前的状态、评价产科处理经过,指导新生儿治疗、评估新生儿预后等有着极其重要的意义,并针对问题对产科、新生儿科提出积极预防干预措施。
Objective: To observe the relationship between p H value of umbilical arterial blood gas and blood glucose in neonates with amniotic fluid meconium-stained. Methods: From May 1, 2013 to June 1, 2014, 484 neonates with different levels of meconium-stained amniotic fluid were enrolled. Among them, 160 were amniotic fluid meconium-stained, 135 were amniotic fluid meconium-stained, 135 were amniotic fluid meconium-stained Fecal pollution Ⅲ ° 189 cases. Results: There was significant difference in p H value and blood glucose among umbilical arterial blood gas indexes. The degree of amniotic fluid meconium staining in Ⅰ ° and Ⅱ ° was in direct proportion to p H value of umbilical arterial blood gas and blood sugar, and Ⅲ ° amniotic fluid meconium-stained newborn Umbilical arterial blood gas index p H value and blood glucose is inversely proportional relationship. Conclusion: There are significant differences in p H value of umbilical arterial blood gas with different levels of amniotic fluid meconium-contaminated neonates and blood glucose results, which are closely related to the degree of neonatal asphyxia and hypoxia, which helps to determine the nature and severity of fetal blood- It is very important for the analysis of the status of the fetus before delivery, evaluation of obstetric treatment, guidance of neonatal treatment, assessment of the prognosis of newborn, and put forward positive preventive interventions for obstetrics and neonatology.
