目的探讨运动平板试验与动态心电图在老年冠心病诊断中的应用价值。方法选择2014年1月—2015年1月收治的80例疑诊为老年冠心病患者,分别进行运动平板试验和动态心电图检查,检查的间隔时间为14 d。计数资料比较采用χ~2检验,P<0.05为差异有统计学意义。以冠状动脉造影为金标准,对比运动平板试验和动态心电图诊断的准确性。结果运动平板试验诊断灵敏度为96.43%,特异度为58.33%,阳性预测值为84.38%,阴性预测值为87.50%;动态心电图诊断灵敏度为67.86%,特异度为58.33%,阳性预测值为79.17%,阴性预测值为43.75%;二者灵敏度、阳性预测值比较差异有统计学意义(均P<0.05)。结论运动平板试验诊断老年冠心病的准确性及灵敏度高于动态心电图。
Objective To investigate the value of treadmill exercise test and Holter in the diagnosis of coronary heart disease in the elderly. Methods From January 2014 to January 2015, 80 patients with suspected coronary heart disease were enrolled in the study. The treadmill test and ambulatory electrocardiogram were performed respectively, the interval time was 14 days. Count data were compared using χ ~ 2 test, P <0.05 for the difference was statistically significant. To coronary angiography as the gold standard, comparative exercise treadmill test and Holter diagnostic accuracy. Results The diagnostic sensitivity of treadmill test was 96.43%, the specificity was 58.33%, the positive predictive value was 84.38%, the negative predictive value was 87.50%. The diagnostic sensitivity of dynamic electrocardiogram was 67.86%, the specificity was 58.33%, the positive predictive value was 79.17% , And the negative predictive value was 43.75%. There was significant difference between the sensitivity and positive predictive value (both P <0.05). Conclusion The accuracy and sensitivity of exercise treadmill test in diagnosis of elderly coronary heart disease are higher than those of dynamic electrocardiogram.
