目的:探讨CD133蛋白在肺神经内分泌癌中的表达及其临床病理意义。方法:采用免疫组化法检测90例肺神经内分泌癌患者癌组织中CD133蛋白表达并分析其与各临床病理指标及预后的关系。结果:CD133蛋白表达定位于癌细胞胞膜及胞质;CD133表达阳性率分别为类癌14.3%(2/14),不典型类癌45.0%(9/20)、大细胞神经内分泌癌55.6%(10/18)和小细胞癌60.5%(23/38),差异有统计学意义,P=0.005。CD133表达癌在Ⅰ期为21.1%(4/19),Ⅱ期56.0%(14/25),Ⅲ期54.8%(23/42),Ⅳ期75.0%(3/4),差异有统计学意义,P=0.015。CD133表达癌在肿瘤直径大小的分布为<5 cm者39.2%(20/51),≥5 cm者61.5%(24/39),差异有统计学意义,P=0.035。CD133表达在淋巴结转移癌为56.3%(40/71),未转移癌为21.1%(4/19),差异有统计学意义,P=0.009。CD133阴性表达癌患者的中位生存时间为55个月,CD133阳性癌的患者中位生存时间为13个月,两者比较差异有统计学意义(Log-rank=35.91,P=0.000 1)。CD133蛋白表达与患者年龄(P=0.191)和性别(P=0.242)无关。结论:CD133蛋白在肺神经内分泌癌的发生、演进过程中起着重要作用,并可作为分子指标监测患者预后。
Objective: To investigate the expression of CD133 protein in pulmonary neuroendocrine carcinoma and its clinicopathological significance. Methods: Immunohistochemistry was used to detect the expression of CD133 protein in 90 patients with pulmonary neuroendocrine carcinoma and its relationship with each clinicopathological parameters and prognosis. Results: The positive rate of CD133 expression was 14.3% (2/14) in carcinoid, 45.0% (9/20) in atypical carcinoid, 55.6% in large cell neuroendocrine carcinoma, (10/18) and small cell carcinoma (60.5%, 23/38), the difference was statistically significant (P = 0.005). The expression of CD133 in stage Ⅰ was 21.1% (4/19), stage Ⅱ 56.0% (14/25), stage Ⅲ 54.8% (23/42), stage Ⅳ 75.0% (3/4), the difference was statistically significant , P = 0.015. The distribution of CD133 was 39.2% (20/51) in patients with tumor size of 5 cm and 61.5% (24/39) in ≥5 cm, the difference was statistically significant (P = 0.035). CD133 expression in lymph node metastasis was 56.3% (40/71), non-metastatic cancer was 21.1% (4/19), the difference was statistically significant, P = 0.009. The median survival time of patients with CD133 negative expression was 55 months. The median survival time of patients with CD133 positive cancer was 13 months. The difference was statistically significant (Log-rank = 35.91, P = 0.000 1). CD133 protein expression was not associated with patient age (P = 0.191) and sex (P = 0.242). Conclusion: The CD133 protein plays an important role in the development and progression of pulmonary neuroendocrine carcinomas and can be used as a molecular marker to monitor the prognosis of patients.
