目的探讨食管鳞癌组织中肿瘤芽孢数及其与临床病理特征和患者预后的关系。方法选取2010年2月至2011年1月进行手术根治的100例食管鳞癌患者,取患者鳞癌组织制作病理切片,苏木精-伊红染色,显微镜下统计每例切片肿瘤芽的数量、分级,分析肿瘤芽与临床病理特征和患者预后的关系。结果在100例食管鳞癌组织中,0级肿瘤芽18例,1级肿瘤芽22例,2级肿瘤芽28例,3级肿瘤芽32例。低分化食管鳞癌患者3级肿瘤芽的比例显著高于中分化及高分化(P<0.01);浸润深度上,黏膜层3级肿瘤芽的比例显著高于深肌层、浅肌层、全肌层(P<0.05);Ⅲ期3级肿瘤芽的比例显著高于Ⅰ期、Ⅱa期、Ⅱb期(P<0.05);有淋巴转移3级肿瘤芽的比例显著高于无淋巴转移(P<0.05)。1级肿瘤芽的3年生存率、5年生存率显著高于2级、3级肿瘤芽(P<0.05),2级肿瘤芽的3年生存率、5年生存率显著高于3级(P<0.05)。结论肿瘤芽的表达与食管鳞癌患者的临床病理特征及预后有关。
Objective To investigate the number of tumor spores in esophageal squamous cell carcinoma and its relationship with clinicopathological characteristics and prognosis. Methods 100 patients with esophageal squamous cell carcinoma who underwent radical operation from February 2010 to January 2011 were selected for pathological examination and hematoxylin and eosin staining. The number of tumor buds in each section was counted under a microscope. Grading, analysis of tumor buds and clinicopathological features and prognosis of patients. Results In 100 cases of esophageal squamous cell carcinoma, there were 18 cases of grade 0 tumor bud, 22 cases of grade 1 tumor bud, 28 cases of grade 2 tumor bud and 32 cases of grade 3 tumor bud. The proportion of grade 3 tumor buds in patients with poorly differentiated esophageal squamous cell carcinoma was significantly higher than that in moderately differentiated and well differentiated patients (P <0.01). At the depth of invasion, the proportion of grade 3 tumor buds in mucosa was significantly higher than that in deep muscular layer and superficial muscle layer (P <0.05). The proportion of grade 3 tumor buds was significantly higher than that in stage Ⅰ, Ⅱa and Ⅱb stages (P <0.05). The proportion of grade 3 tumor buds with lymph node metastasis was significantly higher than that without lymph node metastasis <0.05). The 3-year survival rate and 5-year survival rate of grade 1 tumor buds were significantly higher than those of grade 2 and grade 3 tumor buds (P <0.05). The 3-year survival rates and 5-year survival rates of grade 2 tumor buds were significantly higher than those of grade 3 P <0.05). Conclusion The expression of tumor buds is related to the clinicopathological characteristics and prognosis of patients with esophageal squamous cell carcinoma.
