The doses of antibiotics and other medications need to be adjusted for renal insufficiency. Many physicians rely on chart equations, textbooks or clinical pharmacists for dose adjustments that are difficult to understand and remember when adjusting. This article describes a simple calculation of nephropathy (DREM) system to calculate the dose adjustment interval.
