目的应用超声造影(CEUS)判断兔肝VX2肿瘤射频消融后肿瘤的灭活情况,探讨CEUS在评价肝癌射频消融疗效的可靠性。方法 9例兔VX2肝肿瘤模型作RFA治疗后4d作Sonovue介导的CEUS检查,分析比较各兔子CEUS图片,确定肿瘤部位、数目、边界和肿瘤的坏死程度,选取有代表性的图片作径线测量和勾画出消融灶面积比例并与病理作比较,采用网格分析法计算出CEUS评价RFA疗效的诊断指标。结果9例兔子中8例发现残留肿瘤灶,1例完全消融,残留灶在CEUS上表现为“快进快退”增强特点,消融灶一直呈无增强状态。以超声测量相关径线与病理对比基本无差异,消融灶面积比例分别为0.55±0.27和0.53±0.33,存在高度正相关(R2=0.89,P=0.001),其诊断敏感率、特异性、阳性预测值、阴性预测值和诊断准确率分别为86%、88%、91%、80%和86%。结论CEUS能较可靠评价RFA疗效。
Objective To evaluate the tumor inactivation of rabbit VX2 tumor after radiofrequency catheter ablation by contrast echocardiography (CEUS) and evaluate the reliability of CEUS in assessing the efficacy of radiofrequency ablation of liver cancer. Methods Nine rabbit VX2 liver tumor models were treated with Sonovue-mediated CEUS 4 days after RFA treatment. CEUS images of each rabbit were analyzed and compared to determine the tumor location, number, border and tumor necrosis.
