目的探讨基层医院严重多发伤的早期救治经验与教训,减少死亡率和致残率,提高早期诊断和治疗水平。方法回顾济源钢铁集团有限公司职工医院10年间收治的236例严重多发伤患者的临床资料,分析基层医院严重多发伤早期救治中诊断、急救、手术等问题,提出早期救治的要点。结果本组死亡27例,占11.44%,治愈209例,占88.56%。颅脑损伤为主要死亡原因,其次为大失血。结论抓紧创伤急救的“黄金时间”是抢救成功的关键,提高早期诊断及治疗的准确性,优先解决危及生命的损伤是提高严重多发伤救治成功的重要因素。
Objective To explore the early treatment experience and lessons learned from primary treatment of severe multiple trauma in primary hospitals, to reduce the mortality and morbidity, and to improve the early diagnosis and treatment. Methods The clinical data of 236 patients with severe multiple trauma admitted to the staff hospital of Jiyuan Iron & Steel (Group) Co. Ltd. in 10 years were retrospectively analyzed. The diagnosis, first aid and surgery in the early treatment of severe multiple trauma in primary hospitals were analyzed. The main points of early treatment were put forward. Results The group died of 27 cases, accounting for 11.44%, 209 cases were cured, accounting for 88.56%. Brain injury is the main cause of death, followed by large blood loss. Conclusion Paying close attention to the “prime time” of first aid for trauma is the key to the success of salvage. Improving the accuracy of early diagnosis and treatment and giving priority to resolving life-threatening injuries are important factors in improving the success of salvage in serious trauma.
