An emerging surgical technique for the treatment of internal carotid artery occlusion, intracranial / external anastomosis, was reported by Donaghy and Yasargil in 1967, which at the time heralded major advances in the treatment of cerebrovascular disease. Due to the logical nature of the surgery itself, the anastomosis between the superficial temporal artery and the middle cerebral artery branch is quickly welcomed by the neurosurgery community. It has been shown for the first time that bypass surgery seems to be useful for those with atherosclerotic lesions of the symptomatic internal carotid artery or middle cerebral artery that are not directly amenable to endarterectomy. During the first decade following the introduction of intracranial / extra-arterial bypass surgery, many reports indicate that the mortality and morbidity of this procedure can be quite low and technically achievable with a high degree of patency.
