头颈部大的血管瘤出现出血、溃疡和感染的发病率及死亡率较高,以往采用非手术疗法如放疗、类固醇及硬化剂,疗效不满意,手术结扎供血的血管,仅有暂时疗效。本文报告3例小儿颌面部血管瘤术前栓塞的经验。3例分别诊为下颌管海绵状血管瘤、颈前口底及舌部血管瘤、右下颌血管瘤。均全麻下经股办脉插管,栓塞材料为明胶海绵、Gianturco圈或Ivalon,栓塞下齿槽动脉或舌动脉。栓塞后48~72小时手术,术中出血少,术后情况良好。作者提出栓塞血管是治疗头颈部血管瘤有用的辅助方法。
Hemangiomas with large head and neck hemangiomas have a higher incidence of bleeds, ulcers, and infections, as well as higher mortality. In the past, non-surgical treatments such as radiotherapy, steroids, and sclerosing agents were not satisfactory, and surgical ligation of blood supply vessels had only temporary effects. This article reports the experience of preoperative embolization of 3 cases of pediatric maxillofacial hemangioma. Three cases were diagnosed as cavernous hemangioma of the mandibular canal, anterior cervical floor, tongue hemangioma, and right mandibular hemangioma. Under general anesthesia, the stocks were intubated and the embolization material was gelatin sponge, Gianturco ring or Ivalon, and the inferior alveolar artery or tongue artery was embolized. 48 to 72 hours after embolization, bleeding during the operation was small, and the postoperative condition was good. The authors propose that embolization of blood vessels is a useful auxiliary method for the treatment of head and neck hemangiomas.
