  Diabetes mellitus (DM) is a metabolic disease responsible for about 5% of all deaths each year, which will likely increase by more than 50% in the next 10 years.According to the 2011 report of US Renal Data System, diabetes as a comorbid condition was reported in 44% of incident ESRD patients in the USA.According to a 2010 Census from Brazilian Society of Nephrology (SBN), there are currently 92,091 patients on dialysis treatment in Brazil.The two main causes of end stage renal disease in these patients are systemic hypertension (35,2%) and diabetes mellitus (27,5%).DM leads to micro-and macrovascular disease characterized by endothelial dysfunction and several studies suggest its association with a decreased generation of nitric oxide (NO).DM is also associated to activation of inflammatory cytokines and increased oxidative stress, which with the hyperglycemia are considered the major causes of diabetic nephropathy (DN).Acute physical activity can induce oxidative damage to tissues, because of increased whole body and tissue rates of oxygen consumption, but a routine that includes exercises, enhances antioxidant defenses in different organs.In a recent study we showed that training of diabetic rats during eight weeks resulted in attenuation of diabetes complications, with renal function improvement and reduction of albuminuria and TBARS (an indirect measurement of lipid peroxidation) concentrations, associated with partial recovery of urinary NO and kidney lesion amelioration.Plasmatic insulin concentrations were significantly decreased in DM rats when compared with the respective controls, without differences between sedentary and trained animals, meaning that the kidney protection was not due to exercise induced increase in insulin.This study suggests that aerobic training associated with the control of oxidative stress can be used to delay the progression of diabetic nephropathy.
