  We will discuss the possibilities of telehealth and mHealth interventions to improve diabetes care.At our institution, we created a website for patients and families of Type 1 diabetics that allowed for review of clinic test results, review of educational materials related to these results and posting of questions to their diabetes educator.Fingerstick hemoglobin Alc (HbAlc) testing and periodic use of a continuous glucose monitoring system (CGMS) were offered to all patients.The website, an asynchronous (store-and-forward) telemedicine system, showed patients their HbA1c and CGMS results after each clinic visit.In our trial, fifty-two children were enrolled to use the site over a one year period.For children whose HbA1c was above age recommended guidelines, HbA1c decreased by a mean of 1.4% after six months for those classified as repeat users of the website versus a mean increase of 0.9% for those that were non-users of the site [p=0.03].Subsequent analysis showed users of the site had decreased glycemic variability as shown by reduction in the frequency of symptomatic hypoglycemic events between clinic visits [p=0.03].We will review the advantages and disadvantages of our asynchronous approach to telehealth systems.We will also discuss our ongoing mHealth initiatives to include use of the MedApps HealthPAL device to automate uploading of glucometer readings and the development of our diabetes iPhone app.
