The peptide motilin has a pronounced effect on gastrointestinal motility and is involved in the regulation of the so called "migrating motor complex (MMC)" in the fasted state.The discovery that the long known antibiotic erythromycin is a motilin agonist allowed a further exploration of motilin agonism.When it was found that erythromycin accelerates gastric emptying in diabetic gastroparesis,the motilin receptor became an interesting pharmacological target.More potent erythromycin derivatives (motilides),motilin analogues; or synthetic non-peptide motilin agonists; have been developed but as yet none has made it into the clinic.There are several reasons for this,but perhaps also the area of application has also to be revised.It may be indicated to abandon gastroparesis and to aim at effects on fasted motor activity.Induction of the MMC may reduce exposure of stomach and esophagus to acid secretions and be of benefit in patients with reflux disease and peptic ulcer.It may also help to prevent bacterial overgrowth,a condition which is now considered to play a role in the development of IBS.
