PK/PD与抗菌药物的个体化治疗

抗菌药物与其他药物作用的不同之处在于其对象不仅是机体,还包括引起机体感染的病原微生物,其药理学研究涉及药物、病原微生物、机体三方面.因此药物-机体-病原微生物是决定抗菌药物疗效的三要素,其药代动力学(PK)与药效动力学(PD)的相互关系影响治疗方案的制定.抗菌药物合理使用就是在全面了解患者、致病菌与抗菌药物三者的特点与相互关系的基础上,安全有效地应用抗菌药物,使病人承担最小的风险,最小的支出(最合理的支出),获得最大的治疗效益。PK/PD参数对优化临床给药方案是基于药物在感染部位对病原菌的清除作用有足够的浓度和足够的作用时间,是一个体化方案。个体化治疗兼顾了患者的个体情况和药物的治疗作用,是治疗中最可取的理念。根据不同药物的PK/PD参数与疗效的相关性,可以用参数预测临床疗效,并通过该参数指导制订最佳给药方案,最大限度达到个体化给药。
