Patient Room Visibility and ICU Mortality

  This study tests the association between patient room visibility and mortality in the intensive care unit (ICU).Studies suggest that patient room visibility is associated with clinical outcomes among hospitalized patients.Whether this association also holds true for patients in the ICU has not been rigorously evaluated.We evaluated data on all patients admitted to the medical ICU in one year at a tertiary care hospital.Several computer-based visibility measures of patient rooms were used : visibility of patient head,visibility of patient room,and field of view to nursing station.Visibility of patient head was defined as the percentage of area within the central nursing station from which the patient head could be seen ;visibility of patient room was defined as the percentage of area within the central nursing station that could see the patient room (average value of all patient room grids) ;field of view was defined as the maximum viewing angle from the patient head to the central nursing station.Among the sickest patients (those with APACHE Ⅱ scores >30),field of view was significantly associated with ICU mortality (R2=0.335,F (1,10)=5.03,p=0.049;regression analysis).Field of view accounted for a large proportion of variance (33.5%) in ICU mortality across different rooms.Subtle differences in patient room visibility may have important effects on clinical outcomes.
