For the last 50 years the neurologic evaluation of peripheral nerve disorders has focused primarily on large diameter nerve fibers.From the physical examination which emphasizes muscle strength testing, sensory evaluation of vibration and light touch, and muscle stretch reflexes to the neurophysiology laboratory which concentrates on nerve conduction studies and needle electromyography, to the pathology department which evaluates sural nerve biopsies with light microscopy and osmicated teased fibers, it seems that the neurology profession has been preoccupied with large diameter nerve fibers.
