目的探讨人表皮生长因子受体2(HER2)、切除修复交叉互补基因1(ERCC1)、表皮生长因子受体(EGFR)的表达以及EGFR基因扩增能否预测含铂方案治疗晚期食管鳞癌的疗效。方法回顾性分析57例采用含铂方案进行一线化疗的晚期食管鳞癌患者,应用免疫组化的方法检测HER2、ERCC1和EGFR的表达,并在EGFR表达为弱阳性(++)和强阳性(+++)的患者中(24例)应用荧光原位杂交(FISH)技术检测EGFR基因扩增。结果 HER2、ERCC1和EGFR的表达率分别为15.8%、47.4%和55.3%,24例EGFR(++)和(+++)的患者中EGFR基因扩增为16.9%,未发现HER2、ERCC1和EGFR的表达与化疗的客观缓解率(ORR)、肿瘤进展时间(TTP)和中位生存时间(OS)相关。EGFR基因扩增患者的OS有缩短的趋势。结论 HER2、ERCC1和EGFR的表达可能不能预测含铂方案治疗晚期食管鳞癌的疗效,需要寻找新的疗效预测指标。
Objective To investigate whether the expression of human epidermal growth factor receptor 2 (HER2), excision repair cross-complementary gene 1 (ERCC1), epidermal growth factor receptor (EGFR) and EGFR gene amplification can predict the prognosis of advanced esophageal squamous cell carcinoma Efficacy. Methods Fifty-seven patients with advanced esophageal squamous cell carcinoma who underwent first-line chemotherapy with platinum regimen were retrospectively analyzed. The expressions of HER2, ERCC1 and EGFR were detected by immunohistochemistry. The expressions of EGFR were weak positive (++) and strongly positive +++) (24 cases), EGFR gene amplification was detected by fluorescence in situ hybridization (FISH). Results The expression rates of HER2, ERCC1 and EGFR were 15.8%, 47.4% and 55.3% respectively. The EGFR gene amplification was 16.9% in 24 patients with EGFR (++) and (+++). No HER2, ERCC1, The expression of EGFR correlated with the objective response rate (ORR), tumor progression time (TTP) and median survival time (OS). Patients with EGFR gene amplification have a tendency to have a shortened OS. Conclusions The expression of HER2, ERCC1 and EGFR may not predict the efficacy of platinum-containing regimen in the treatment of advanced esophageal squamous cell carcinoma, and new prognostic indicators need to be found.
