目的 :评价角膜厚度在高眼压症眼压测量中的作用影响。方法 :选取 34例 (6 8眼 )高眼压症患者 (眼压≥ 2 1mm Hg,≤ 35 mm Hg,正常视野 ,正常视神经乳头 ,无角膜水肿等病变 )均为复旦大学医学院眼耳鼻喉科医院眼科门诊患者 ,年龄 6~ 6 5岁 (19.70±13.5 2 ) ,平均 2 0岁 ,男性 18例 ,女性 2 0例。由专人应用 Goldmann压平眼压计测量眼压 ,由专人应用 Sonoscan Model40 0 0 APA型超声测量中央角膜厚度。结果 :34例 6 8眼眼压范围 2 1~ 35 mm Hg,平均 2 4.892± 2 .885 mm Hg;中央角膜厚度 0 .5 2 3~0 .6 37mm,平均 0 .5 88± 0 .0 2 8mm。应用 SPSS软件包经 K—S检验二组数据 P值分别为 0 .30 6 1、 0 .842 2均呈正态分布 ,应用Pearson s相关统计法 ,二组数据呈正相关 ,相关系数为 0 .2 82 4,P=0 .0 2 (P<0 .0 5 )。根据本组病例可得出一相关方程式 :Y(眼压 ) =42 .40 1X(中央角膜厚度 )。即每 0 .0 2 36 mm的中央角膜厚度变化可有 1mm Hg的压平眼压值改变。结论 :中央角膜厚度与眼压测量值呈正相关 ,高眼压症患者眼压值的升高与中央角膜厚度的变化有关 ,揭示高眼压症患者的真正眼压可能是正常的 ,只是由于中央角膜厚度的变异使眼压测量值偏高。
Objective: To evaluate the effect of corneal thickness on intraocular pressure measurement in ocular hypertension. Methods: 34 patients (68 eyes) with ocular hypertension (intraocular pressure ≥ 21 mm Hg, ≤ 35 mm Hg, normal field of vision, normal optic nerve head, no corneal edema and other lesions) were Fudan University School of Medicine ENT Hospital ophthalmology outpatient, aged 6 to 65 years (19.70 ± 13.52), average 20 years, 18 males and 20 females. The intraocular pressure was measured by hand using a Goldmann flattening tonometer. The central corneal thickness was measured by hand using Sonoscan Model 40 0 APA-type ultrasound. Results: The intraocular pressure range of 34 eyes was 21 ~ 35 mm Hg in 48 eyes, with an average of 4.892 ± 2.885 mm Hg. The central corneal thickness was 0.52 3 ~ 0.367 mm, with an average of 0.588 ± 0 .0 2 8mm. SPSS software package by K-S test P value of the two groups were 0 .30 6 1, 0. 842 2 were normal distribution, the application of Pearson’s correlation statistics, the two groups of data was positively correlated, the correlation coefficient of 0. 2 82 4, P = 0 .0 2 (P <0 .0 5). According to this group of patients can be drawn a correlation equation: Y (intraocular pressure) = 42 .40 1X (central corneal thickness). The change of the central corneal thickness per 0.252 mm may change with the value of 1 mm Hg. Conclusion: There is a positive correlation between central corneal thickness and intraocular pressure (IOP) measurement. The elevation of intraocular pressure (IOP) in patients with ocular hypertension is related to the change of central corneal thickness. It is suggested that the true IOP in patients with ocular hypertension may be normal, Variation in corneal thickness causes high IOP measurements.
