作者报道了4例骨骼未发育成熟患脊髓肿瘤的病例,均具有早期独特症状:病人头部处于前屈位且不能抬起,当颈部被动伸展时伴疼痛,此即“低头征”。此征对于脊髓肿瘤或其它脊髓病变的早期诊断有帮助。 例1,男孩,98/12岁,诉背部疼痛且头部无法抬起。脊髓MRI扫描显示硬膜内肿块从胸_3延伸至脊髓圆锥,伴上方瘘管形成。组织学检查证实为Ⅱ级星形细胞瘤,原纤维型。例2,女孩,21/12岁,走路时足尖朝外,有运动失调性步态和颈部处
The authors reported four cases of unmatured bone tumors in the spinal cord, all with early and unique symptoms: the patient’s head is in flexion and can not be lifted, and pain is present when the neck is passively stretched. This sign is helpful for the early diagnosis of spinal cord tumors or other myelopathy. Example 1, a boy, 98/12 years old, complains of back pain and can not lift his head. Spinal MRI scans showed an intradural mass extending from the thoracic-3 to the spinal cone, with upper fistula formation. Histological examination confirmed as grade Ⅱ astrocytoma, fibrillar type.
