目的 探讨男性乳腺腺样囊性癌的临床病理特征、诊断与鉴别诊断.方法 观察1例罕见男性乳腺原发的腺样囊性癌的临床、组织学、免疫表型及分子病理学特征,结合文献分析其病理特征及鉴别诊断.结果 患者男性,27岁,无意间发现左侧乳晕下方乳腺条索状包块2年余.B超示左侧乳腺内见一低回声区,界欠清,未见明显彩色血流信号,考虑男性乳腺发育.大体肿物境界不清,切面灰黄灰红,质地中等.镜下肿瘤组织呈筛孔状、腺样分布,浸润性生长,肿瘤细胞圆形、卵圆形,胞质少,核分裂象可见.免疫组化:肿瘤细胞CD117、MYB、CK14、CK5/6、p63及SMA(+),而ER、PR及CerbB-2均(-).分子病理FISH检测MYB基因重组,MYB基因断裂阳性.结论 男性乳腺原发腺样囊性癌少见,经形态学、免疫组化标记及分子病理学相结合可明确诊断.
