目的通过比较2型糖尿病患者与健康者血清中TNF-a水平的差异,以及TNF-a与糖代谢状况的关系,探讨2型糖尿病患者易患牙周病的可能原因。方法测定36例2型糖尿病患者和36例全身健康者血清中TNF-a水平含量,用高效液相层析法测定2型糖尿病患者的糖化血红蛋白水平。结果 2型糖尿病患者血清中TNF-a水平含量高于健康对照组(P<0.01);糖尿病合并牙周病组血清中TNF-a水平含量与糖化血红蛋白(HbA1c)水平有一定的相关性,随着HbA1c水平的下降,TNF-a水平逐渐改善,对其进行直线相关分析,结果表明两者无相关性(P<0.05)。结论糖尿病患者因糖代谢等全身反应异常,血清中TNF-a含量显著升高,导致易患牙周病,可能是2型糖尿病合并牙周病的病理基础之一。
Objective To explore the possible causes of periodontal disease in type 2 diabetic patients by comparing the difference of serum TNF-a level between type 2 diabetic patients and healthy subjects and the relationship between TNF-α and glucose metabolism. Methods The levels of TNF-a in 36 patients with type 2 diabetes mellitus and 36 healthy individuals were measured. The levels of HbA1c in patients with type 2 diabetes mellitus were determined by high performance liquid chromatography. Results Serum levels of TNF-α in patients with type 2 diabetes mellitus were significantly higher than those in healthy controls (P <0.01). There was a positive correlation between serum TNF-a levels and HbA1c levels in diabetic patients with periodontal disease With the decrease of HbA1c level and the gradual improvement of TNF-a level, linear correlation analysis showed that there was no correlation between them (P <0.05). Conclusion Diabetic patients with abnormal systemic response to glucose metabolism and serum TNF-a levels were significantly increased, leading to susceptibility to periodontal disease, which may be one of the pathological basis of type 2 diabetes mellitus with periodontal disease.
