糖尿病患者中,全身可出现各种神经损害。损害部位可发生在脑(血管损害、低血糖)、脊髓、脊髓根、末梢神经及运动神经、感觉神经、植物神经等,由于损害部位不同,将引起的症状也是多种多样的,因而关于神经系统损害提倡多种分类方法。关于糖尿病性神经系统损害的原因,有血管学说、脂质代谢学说、多元醇代谢学说、肌醇学说等。糖尿病性神经损害因无特异性症状和体征,临床诊断是用排除法。检查末梢神经机能异常可以应用各种方法,我们用测定跟腱反射持续时间,神经亢进传导速度,振动感觉阈值等方法,观察末梢神经机能异常程度的变化和进展。
