目的:了解我国乡村两级卫生人员的现状,同时从人口、地域空间的角度评价其配置的公平性,发现存在的问题并提出相应的建议。方法:按照分层抽样的方法,在全国选择400个县级行政单位进行调查,借助洛伦兹曲线和基尼系数对乡村两级卫生人员配置的公平性进行测量和评价。结果:乡村两级卫生人员中执业(助理)医师的比例仍然较低;乡村两级卫生人员配置的人口公平性较好,地理不公平现象较为严重。建议:加强乡村两级卫生人才队伍的稳定性;加强对乡村两级卫生人员的继续教育和技能培训;继续加大对西部地区的扶持力度;科学实施区域卫生规划,提高配置的公平性。
OBJECTIVE: To understand the status quo of health personnel at the rural and urban levels in our country, evaluate the fairness of its allocation from the perspective of population and geographical space, find out the existing problems and make corresponding suggestions. Methods: According to the method of stratified sampling, 400 county-level administrative units were selected in the whole country for investigation. The Lorenz curve and Gini coefficient were used to measure and evaluate the fairness of rural health personnel at two levels. Results: The proportion of practicing (assistant) physicians at the two levels in rural areas is still relatively low. The distribution of health workers at rural and urban levels is fairly fair and the geographical inequities are more serious. Suggestions: to strengthen the stability of the health personnel at the village and village levels; to strengthen the continuing education and skills training of rural health personnel at both levels; to continue to increase support for the western region; and to scientifically implement regional health plans to enhance the fairness of the allocation.
