目的:研究EB病毒(EBV)感染患儿SH2D1A mRNA表达情况,探讨SH2D1A mRNA表达改变与疾病的关系及其意义。方法:选取2014年10月至2015年2月湖南省儿童医院肝病中心收治的初诊为EB病毒感染的患儿,选取同期年龄、性别与EB病毒感染组匹配的健康体检儿童为对照组。检测EB病毒感染组和对照组的SH2D1A mRNA表达水平,计算基因相对表达量,检测血生化指标。结果:EB病毒感染组共32例,其中普通传染性单核细胞增多症(普通组)20例,重症传染性单核细胞增多症(重症组)9例,慢性活动性EBV感染(慢性组)1例,EBV相关噬血淋巴组织细胞增多症(噬血组)2例;对照组10例。各组与对照组SH2D1A mRNA的相对中位表达量分别为25.78、40.14、38.72、18.27、4.65(H=10.68,P<0.05),EB病毒感染组患儿的SH2D1A mRNA表达水平显著高于对照组,重症组、慢性组SH2D1A mRNA表达水平较普通组、噬血组升高(P<0.05),但重症组与慢性组比较差异无统计学意义(P>0.05)。SH2D1A mRNA表达水平与初诊时最低的纤维蛋白原、CD4+、CD4/+CD8+水平呈负相关(P均<0.05),与血清铁蛋白、丙氨酸氨基转移酶、乳酸脱氢酶、甘油三酯、EBV-DNA水平无相关性。结论:EBV感染患儿外周血细胞SH2D1A mRNA表达水平显著升高,造成机体细胞免疫功能紊乱。
Objective: To investigate the expression of SH2D1A mRNA in children with Epstein-Barr virus (EBV) infection and to explore the relationship between SH2D1A mRNA expression and disease and its significance. Methods: From January 2014 to February 2015, children with newly diagnosed Epstein-Barr virus (EBV) infection admitted to the Hepatology Center of Hunan Provincial Children’s Hospital were enrolled. Children of healthy subjects matched for age, sex and EBV infection were selected as control group. The expression of SH2D1A mRNA in Epstein-Barr virus (EBV) infection group and control group was detected, and the gene expression level was calculated. Blood biochemical parameters were detected. Results: There were 32 cases of EBV infection, including 20 cases of common infectious mononucleosis (general group), 9 cases of severe infectious mononucleosis (severe group), 9 cases of chronic active EBV infection (chronic group) 1 case, EBV-associated hemophagocytic metaplasia (hemophagocytic group) 2 cases; control group 10 cases. The relative median expression of SH2D1A mRNA in each group was 25.78, 40.14, 38.72, 18.27 and 4.65 respectively (H = 10.68, P <0.05). The expression of SH2D1A mRNA in EBV-infected children was significantly higher than that in the control group The expression of SH2D1A mRNA in severe group and chronic group was higher than that in normal group and hemophagocytic group (P <0.05), but there was no significant difference between severe group and chronic group (P> 0.05). The expression of SH2D1A mRNA was negatively correlated with the lowest level of fibrinogen, CD4 + and CD4 / + CD8 + in newly diagnosed cases (all P <0.05), but not with serum ferritin, alanine aminotransferase, lactate dehydrogenase, triglyceride , EBV-DNA level no correlation. Conclusion: The expression of SH2D1A mRNA in peripheral blood cells of children with EBV infection is significantly increased, resulting in the disorder of cellular immune function.
