目的探讨热休克蛋白90α(HSP90α)、基质金属蛋白酶9(MMP-9)在胆管癌组织中的表达及其临床意义。方法选取2014年4月至2016年4月收治的50例胆管癌行手术切除患者的存档组织蜡块,根据胆管癌临床病理资料将胆管癌组织分为有浸润转移组(n=35)和无浸润转移(n=15),采用免疫组化PV二步法研究检测MMP-9及HSP90α蛋白质在两组组织中的表达情况。分析HSP90α与MMP-9的相关性,并比较HSP90α与MMP-9诊断胆管癌的效能。结果 MMP-9在有浸润转移组中阳性表达率为77.14%(27/35),明显高于在无浸润转移组中的阳性表达率33.33%(5/15)(P<0.05);HSP90α在有浸润转移组中的阳性表达率为82.86%(29/35),明显高于在无浸润转移组中的阳性表达率26.67%(4/15)(P<0.05);MMP-9的表达与胆管癌临床分期(TNM)、组织分化程度及门静脉浸润和淋巴结转移呈正相关(P均<0.05),与肿瘤大小无关(P>0.05);HSP90α在胆管癌组织中随着癌组织的分化程度的降低、门静脉浸润和淋巴结转移的发生及TNM分期的增加,阳性表达率升高(P均<0.05),与肿瘤大小无关(P>0.05)。结论 HSP90α、MMP-9表达水平与胆管癌的发生、侵袭以及转移有关,且两者之间起相互协同促进作用,检测其在胆管癌组织中的表达具有一定临床诊断价值。
Objective To investigate the expression of heat shock protein 90α (HSP90α) and matrix metalloproteinase 9 (MMP-9) in cholangiocarcinoma and its clinical significance. Methods Fifty paraffin-embedded specimens of paraffin-embedded specimens of cholangiocarcinoma were collected from April 2014 to April 2016. According to the clinicopathological data of cholangiocarcinoma, cholangiocarcinoma tissues were divided into two groups: infiltration and metastasis group (n = 35) (N = 15). The expression of MMP-9 and HSP90α protein in two groups was detected by immunohistochemistry PV two-step method. The correlation between HSP90α and MMP-9 was analyzed and the diagnostic efficacy of HSP90α and MMP-9 in cholangiocarcinoma was compared. Results The positive expression rate of MMP-9 in invasive infiltration group was 77.14% (27/35), which was significantly higher than that in non-infiltrating metastasis group (33.33%, 5/15) (P <0.05) The positive rate of invasion and metastasis was 82.86% (29/35), which was significantly higher than that in non-metastasis group (26.67%, 4/15) (P <0.05) The clinical stage (TNM), histological grade, portal vein infiltration and lymph node metastasis of cholangiocarcinoma were positively correlated (P <0.05), but not with the size of tumor (P> 0.05). The expression of HSP90α in cholangiocarcinoma (P <0.05), but not with the size of the tumor (P> 0.05). Conclusions The expression of HSP90α and MMP-9 is related to the occurrence, invasion and metastasis of cholangiocarcinoma, and they play a synergistic role in promoting the expression of HSP90α and MMP-9 in cholangiocarcinoma. It is of clinical significance to detect the expression of HSP90α and MMP-9.
