目的:探讨不同部位封堵左前降支(left anterior descending artery,LAD)对猪急性心肌梗死(acute myocardial infarction,AMI)模型恶性心律失常发生情况及其与QT间期、校正后QT间期(QTc间期)的关系。方法:选择22头苏中幼猪,采用介入法球囊封堵猪LAD建立AMI动物模型。根据LAD封堵造模的部位,将动物随机分为3组,其中假手术对照组(Sham组,不封堵)4只,LAD下1/3处封堵组和LAD中部封堵组各9只,观察各组在造模过程中恶性心律失常(室性心动过速或心室颤动)的发生率,造模成功后记录并分析各组心电参数指标。结果:LAD中部封堵组心室颤动发生率显著高于下1/3处封堵组(100.00%vs33.33%,P<0.05)。造模成功后,LAD中部封堵组QT间期同下LAD 1/3处封堵组比较,差异无统计学意义[(362.11±29.18)ms vs(338.78±22.65)ms,P>0.05];而LAD中部封堵组QTc间期较下LAD 1/3处封堵组延长,差异具有统计学意义[(462.67±27.55)ms vs(425.56±21.96)ms,P<0.01]。结论:猪急性心肌梗死后恶性心律失常发生与QTc间期延长具有一定的相关性。
OBJECTIVE: To investigate the incidence of malignant arrhythmia in pigs with acute myocardial infarction (AMI) model by occlusion of left anterior descending artery (LAD) at different sites and its relationship with QT interval, QTc Interval) of the relationship. Methods: Twenty-two young and middle-aged pigs were enrolled in this study. AMI animal models were established by balloon occlusion of pigs with interventional methods. According to the site of occlusion of LAD, the animals were randomly divided into 3 groups: sham operation control group (sham group, no occlusion) 4, LAD 1/3 occlusion group and LAD middle occlusion group 9 Only, observe the incidence of malignant arrhythmia (ventricular tachycardia or ventricular fibrillation) in each group in the modeling process, record and analyze ECG parameters of each group after successful modeling. Results: The incidence of ventricular fibrillation in the middle occlusion group was significantly higher than that in the lower 1/3 occlusion group (100.00% vs33.33%, P <0.05). After successful modeling, there was no significant difference in the QT interval between the middle LAD occlusion group and the LAD 1/3 occlusion group [(362.11 ± 29.18) ms vs (338.78 ± 22.65) ms, P> 0.05]. However, the QTc interval in the middle LAD occlusion group was longer than that in the LAD 1/3 occlusion group. The difference was statistically significant (462.67 ± 27.55) ms vs (425.56 ± 21.96) ms, P <0.01. Conclusion: The incidence of malignant arrhythmia after acute myocardial infarction in pigs has some correlation with QTc prolongation.
