目的研究伴中央颞区棘波的儿童良性癫痫(BECT)患儿经奥卡西平单药治疗6个月后其注意网络的变化。方法运用注意网络测试工具(ANT)对服用奥卡西平治疗6个月前后的BECT患儿进行注意网络测试。结果在奥卡西平治疗6个月过程中,76.1%的BECT患儿未有发作,13.0%的患儿发作次数较用药前减少≥50%。71.8%的患儿在奥卡西平治疗6个月后脑电图异常放电得到缓解。BECT患儿经奥卡西平治疗6个月后(T1阶段)注意网络总反应时(876.00±186.68)ms低于治疗前基线水平(T0阶段)(956.76±190.38)ms,差异有显著统计学意义(t=4.111,P=0.000)。T1阶段注意网络检查正确率(97.46±3.20)ms高于T0阶段(95.76±5.38)ms,差异有显著统计学意义(t=-2.578,P=0.013)。警觉网络功能效率比较,T1阶段73(40,99)ms高于T0阶段45(17,84)ms,差异有统计学意义(z=-2.125,P=0.034);定向网络效率T1阶段32(-6,71)ms高于T0阶段3(-27,57)ms,差异有统计学意义(z=-2.213,P=0.027)。治疗前后执行网络功能效率差异无明显统计学意义(P>0.05)。结论奥卡西平可改善BECT患儿的临床发作及脑电图异常放电并可改善患儿注意网络功能,特别是警觉网络功能。
Objective To investigate the change of attention network in children with benign epilepsy (BECT) with central temporal spikes after 6 months of oxcarbazepine monotherapy. Methods The Attention Network Test Tool (ANT) was used to pay attention to the network tests on children with BECT 6 months after taking oxcarbazepine. Results During the 6 months of oxcarbazepine treatment, 76.1% of children with BECT had no seizures and 13.0% of children had seizures ≥50% less than those before treatment. 71.8% of children underwent oxcarbazepine treatment 6 months after the abnormal discharge of EEG was relieved. At 6 months after treatment with oxcarbazepine (T1 stage), the total network response time (876.00 ± 186.68) ms in BECT children was lower than that before treatment (956.76 ± 190.38) ms, the difference was statistically significant (t = 4.111, P = 0.000). T1 phase pay attention to the network check the correct rate (97.46 ± 3.20) ms higher than the T0 stage (95.76 ± 5.38) ms, the difference was statistically significant (t = -2.578, P = 0.013). The functional efficiency of the alerting network was higher than that of the T0 stage 45 (17,84) ms at 73 (40,99) ms in T1 stage (z = -2.125, P = 0.034) -6,71) ms higher than T0 stage 3 (-27,57) ms, the difference was statistically significant (z = -2.213, P = 0.027). Before and after treatment, there was no significant difference in the functional efficiency of executive networks (P> 0.05). Conclusions Oxcarbazepine can improve the clinical onset and abnormal discharge of EEG in children with BECT and improve the children’s attention to the network function, especially the alerting network function.
