Juvenile diabetes mellitus (IDDM) occurs in adolescent diabetes and autoantibodies are detectable in the patient’s serum, suggesting an autoimmune disease that accounts for a large proportion of the genetic predisposition. In recent years, the understanding of the relationship between IDDM and HLA class II antigens has been further clarified. This article briefly discusses the relationship between the two and its related mechanisms. Abnormality of HLA-class II antigens in IDDM Presently there is less knowledge of autoimmunity of IDDM than on autoimmune thyroid disease (ATD), mainly due to the great difficulties of in vitro and in vivo studies of Langerhans islet cells in IDDM. However, class II antibodies are aberrantly expressed on the beta cells of diabetic insulitis
