目的:回顾性分析住院患儿抗菌药物的使用情况,促进临床合理用药水平的持续改进。方法:从我院2015年7月至2016年6月的归档住院病历中随机抽取样本进行分析,重点点评抗菌药物的使用情况,结果按2015年7-12月和2016年1-6月两阶段分别记录并进行比较。结果:2015年7-12月共抽取病历1 430份,其中,930份(65.03%)使用了抗菌药物,抗菌药物应用不合理比例4.09%(38/930);2016年1-6月共抽取病历1 465份,其中,837份(57.13%)使用了抗菌药物,抗菌药物应用不合理比例11.35%(95/837)。抗菌药物不合理使用情况主要表现为给药时机不适宜、特殊级抗菌药物使用不规范、疗程不适宜、联合用药不适宜、抗菌药物品种选择不适宜、用药指征不明确、用法用量不适宜。2015年7-12月不合理用药现象主要为疗程不适宜和联合用药不适宜;2016年1-6月不合理用药现象主要为给药时机不适宜、特殊级抗菌药物使用不规范。结论:我院住院抗菌药物合理用药水平有待提高,须加强对医师、药师的培训,开展持续督查工作,实施奖惩措施,促进抗菌药物合理使用。
OBJECTIVE: To retrospectively analyze the use of antibacterials in hospitalized children and to promote the continual improvement of clinical rational drug use. In July-December 2015, unreasonable drug use was mainly due to inadequate medication and inappropriate combination therapy. In January-June 2016, inappropriate drug use was mainly due to improper timing of delivery and non-standard use of special antimicrobial drugs. Conclusion: The rational use of antibacterials for hospitalization in our hospital needs to be improved. Training of physicians and pharmacists should be strengthened. Continuous supervision should be carried out, incentives and punishments should be implemented to promote the rational use of antibacterials.
