地高辛中毒可能危及生命。已有报道降胆宁与消胆胺通过阻止药物肠肝再循环从而提高地高辛与洋地黄毒甙的清除率。作者报道了1例应用消胆胺成功治疗地高辛中毒。男性患者,94岁,因3天前发生恶心、呕吐、意识模糊及胃纳差入院。患者有多年动脉硬化性心血管病史,曾因外周血管病需行双侧膝关节以上截肢术.有充血性心衰及慢性肾功能衰竭。入院前采用硝酸甘油软膏贴胸壁,每6小时1次;速尿每日口服120mg,地高辛每日口服0.125mg。入院后首次心电图示:窦性心动过缓,室率42次/min,单源性室性早搏,无急性心肌缺血或坏死变化。患者表现
Digoxin poisoning can be life-threatening. It has been reported that carbamazepine and cholestyramine improve the clearance of digoxin and digoxigenin by preventing the drug enterohepatic recirculation. The authors reported the successful administration of cholestyramine in the treatment of digoxin poisoning. Male patient, 94 years old, was hospitalized for nausea, vomiting, confusion, and poor appetite during 3 days. Patients with many years of history of arteriosclerotic cardiovascular disease, had had due to peripheral vascular disease requiring bilateral knee amputation. Congestive heart failure and chronic renal failure. Before admission to nitroglycerin ointment affixed to the chest wall, once every 6 hours; furosemide daily oral 120mg, digoxin daily oral 0.125mg. The first ECG after admission showed: sinus bradycardia, room rate 42 beats / min, single ventricular premature beats, no acute myocardial ischemia or necrosis changes. Patient performance
