目的通过检测高胆红素血症新生儿血清中总胆红素和肌钙蛋白、乳酸脱氢酶、肌酸激酶同工酶等的改变观察高胆红素对心肌的影响,探讨新生儿高胆红素血症是否存在对心肌的损害。方法对100例高胆红素血症新生儿进行肝功能和肌钙蛋白、心肌酶谱检测。结果高胆红素血症患儿治疗前、后肌钙蛋白、心肌酶谱改变均具有高度显著性差异(P<0.01),即治疗前、后肌钙蛋白转阴。总胆红素、心肌酶谱比较:心肌酶谱的升高与血清胆红素升高有关,差异有显著意义(P<0.01)。结论新生儿高胆红素血症可能造成心肌损害,但随着胆红素血症的治愈心肌酶逐渐降至正常,不一定造成心肌细胞的不可逆损害。
Objective To detect the changes of serum total bilirubin, troponin, lactate dehydrogenase and creatine kinase isoenzyme in neonates with hyperbilirubinemia, and to observe the effect of hyperbilirubin on myocardium and explore the relationship between neonatal hyperbilirubinemia Whether there is bilirubin hyperlipidemia of myocardial damage. Methods 100 cases of newborns with hyperbilirubinemia were tested for liver function, cardiac troponin and myocardial enzymes. Results Before and after treatment, the levels of troponin and myocardial enzymes in children with hyperbilirubinaemia had highly significant difference (P <0.01), that is, troponin turned negative before and after treatment. Total bilirubin, myocardial enzymes: myocardial enzymes increased serum bilirubin, the difference was significant (P <0.01). Conclusion neonatal hyperbilirubinemia may cause myocardial damage, but with the cure of bilirubin myocardial enzyme gradually decreased to normal, may not cause irreversible damage to myocardial cells.
