目的观察依达拉奉在破裂脑动脉瘤急性期夹闭术后脑血管痉挛和脑缺血的疗效。方法破裂脑动脉瘤患者68例于发病后72h内施行动脉瘤夹闭术。术后随机分为两组,给予常规治疗;治疗组在对照组的治疗基础上加用依达拉奉2周。分别于术后1、3、7、14d用经颅多普勒超声(TCD)、头颅CT检查,术后3个月行格拉斯哥预后评分(GOS),评价疗效。结果治疗组第3、7天TCD测定的大脑中动脉血液流速明显低于对照组(P<0.05)。治疗组脑缺血的发生率明显低于对照组。治疗组总有效率88.5%,明显高于对照组的71.2%(P<0.05)。结论依达拉奉防治破裂颅内动脉瘤术后脑血管痉挛和脑缺血的效果明显。
Objective To observe the curative effect of edaravone on cerebral vasospasm and cerebral ischemia after acute occlusion of ruptured cerebral aneurysm. Methods A total of 68 patients with ruptured cerebral aneurysm underwent aneurysm clipping within 72 hours after onset. The patients were randomly divided into two groups and were given routine treatment. The treatment group was given edaravone for 2 weeks on the basis of the treatment of the control group. Transcranial Doppler ultrasound (TCD) and cranial CT were performed at 1, 3, 7, and 14 days after operation, and Glasgow Outcome Scale (GOS) was performed at 3 months after operation to evaluate the curative effect. Results The blood flow velocity of middle cerebral artery measured by TCD on the 3rd and 7th day in the treatment group was significantly lower than that in the control group (P <0.05). The incidence of cerebral ischemia in the treatment group was significantly lower than that in the control group. The total effective rate was 88.5% in the treatment group, which was significantly higher than that in the control group (71.2%, P <0.05). Conclusion Edaravone is effective in treating postoperative cerebral vasospasm and cerebral ischemia after ruptured intracranial aneurysm.
