目的研究nm23-H1基因在口腔鳞癌中的蛋白表达,试探讨其与口腔鳞癌颈淋巴结转移的关系及意义。方法采用免疫组化SABC法检测75例口腔鳞癌中nm23-H1蛋白染色,利用卡方检验及多因素Logistic逐步回归进行统计分析。结果nm23-H1蛋白染色阳性信号主要定位于肿瘤和上皮细胞胞浆中。淋巴结转移组nm23-H1,蛋白高染率为15.15%(5/33),无淋巴结转移组高染率为52.38%(22/42),两组间有极显著差异(P<0.01)。nm23-H1蛋白染色水平与病理分级有关(P<0.05),同肿瘤大小、部位、患者的年龄、性别无关。多因素分析发现颈淋巴结转移与nm23-H1蛋白表达水平关系最密切(OR=0.1846)。结论nm23-H1基因的蛋白表达水平高低可能是影响口腔鳞癌颈淋巴结转移的重要因素,检测nm23-H1蛋白的表达水平可能对临床判断和预测口腔鳞癌的转移有一定意义。
Objective To study the expression of nm23-H1 gene in oral squamous cell carcinoma and to explore its relationship with cervical lymph node metastasis in oral squamous cell carcinoma. Methods The immunohistochemical SABC method was used to detect the nm23-H1 protein in 75 cases of oral squamous cell carcinoma. Statistical analysis was performed by means of chi-square test and multivariate Logistic stepwise regression. Results nm23-H1 protein positive staining signal mainly located in the cytoplasm of tumor and epithelial cells. The rate of nm23-H1 protein in the lymph node metastasis group was 15.15% (5/33), and the high staining rate in the group without lymph node metastasis was 52.38% (22/42) (P < 0.01). The level of nm23-H1 protein staining was correlated with pathological grade (P <0.05), but not with tumor size, location, patient's age and gender. Multivariate analysis showed that cervical lymph node metastasis was closely related to nm23-H1 protein expression (OR = 0.1846). Conclusion The expression of nm23-H1 gene may play an important role in the cervical lymph node metastasis of oral squamous cell carcinoma. Detecting the expression of nm23-H1 may be of clinical significance and prognostic significance for the metastasis of oral squamous cell carcinoma.
