目的探讨阑尾黏液性囊腺瘤的临床特点及诊断治疗。方法回顾性分析2002年1月—2011年4月收治的6例阑尾黏液性囊腺瘤的临床资料,对其临床表现、检查结果和手术治疗方法进行分析。结果该病临床表现无特异性,通常是在对右下腹囊性包块或手术切除的炎性阑尾病检时发现,手术治疗效果良好。6例中3例无明显临床症状而以体检发现右下腹囊性肿块入院;2例以突发右下腹痛,误诊为急性阑尾炎入院;1例以右中下腹隐痛不适伴大便习惯改变,误诊为结肠肿瘤入院。1例术前诊断为阑尾黏液性囊腺瘤;2例行单纯阑尾切除术,术后病检为阑尾黏液性囊腺瘤;1例行回盲部切除术及2例行右半结肠切除术,术后病检均为阑尾黏液性囊腺癌;1例因阑尾切除后病检示阑尾黏液腺癌,累积全层,而再次行右半结肠切除术。6名患者随访2~6年,均情况良好。结论阑尾黏液性囊腺瘤发病隐蔽,术前诊断困难,确诊依赖于病理检查。手术完整切除是其唯一有效的治疗方法。
Objective To investigate the clinical features and diagnosis of mucinous cystadenoma in the appendix. Methods The clinical data of 6 cases of appendiceal mucinous cystadenoma admitted from January 2002 to April 2011 were analyzed retrospectively. The clinical manifestations, examination results and surgical treatment were analyzed. Results The clinical manifestations of the disease non-specific, usually in the right lower quadrant cystic mass or surgical excision of the inflammatory appendicitis was found during the operation, the effect is good. 6 cases in 3 cases without obvious clinical symptoms and physical examination found right lower quadrant cystic mass admission; 2 cases of sudden right lower abdominal pain, misdiagnosed as acute appendicitis admitted to hospital; 1 case of right lower abdomen pain and discomfort with bowel habits change, misdiagnosed as Colon cancer hospitalized. One case was diagnosed as appendectomy mucinous cystadenoma. Two cases underwent simple appendectomy and postoperative pathological examination was appendiceal mucinous cystadenoma. One case underwent ileocecal resection and two cases underwent right hemicolectomy , Postoperative pathological examination were appendix mucinous cystadenocarcinoma; 1 case of appendectomy due to appendectomy mucinous adenocarcinoma, the accumulation of full-thickness, and again the right colon resection. Six patients were followed up for 2 to 6 years, all in good condition. Conclusion Appendectomy mucinous cystadenoma occult, preoperative diagnosis difficult, diagnosis depends on the pathological examination. Complete surgical resection is the only effective treatment.
