目的:研究大肠癌组织中的细胞粘附分子D44v6表达及反应强度与肿癌发生、分化、转移和预后的关系。方法:应用免疫组化法和计算机图象分析技术,对90例大肠届组织进行CD44v6 表达和反应强厦定量检测,并对其中53例患者进行随访。结果:68例大肠癌呈CD44v6阳性表达(75.6%)。CD44v6阳性物质主要分布于大肠癌细胞细胞膜和细胞质。图像分析,CD44v6阳性细胞平均积分光密度值在低分化腺癌显著高于高、中分化腺癌和粘液腺癌(P<0.05),伴有浆膜浸润和淋巴结转移者显著高于无浆膜浸润和无淋巴结转移者(P<0.05),在获得5年以上随访材料中,死亡病例显著高于生存病例(P<0.05)。结论:CD44v6反应强度在大肠癌发生、恶性程度、预测转移和客观估测患者预后中具有着重要指导意义。
OBJECTIVE: To investigate the relationship between the expression and response intensity of cell adhesion molecule D44v6 and the oncogenesis, differentiation, metastasis and prognosis of colorectal cancer. METHODS: Immunohistochemistry and computer image analysis techniques were used to quantitatively detect CD44v6 expression and response intensity in 90 cases of large intestine tissues, and 53 of them were followed up. Results: 68 cases of colorectal cancer were positive for CD44v6 (75.6%). CD44v6 positive substances are mainly distributed in the cell membrane and cytoplasm of colorectal cancer cells. Image analysis showed that the mean integrated optical density of CD44v6-positive cells was significantly higher in poorly differentiated adenocarcinoma than in high-, moderately-differentiated adenocarcinoma and mucinous adenocarcinoma (P<0.05). The plasma membrane infiltration and lymph node metastasis were significantly higher than those without. In patients with plasma membrane infiltration and no lymph node metastasis (P<0.05), deaths were significantly higher than those in survivors (P<0.05). Conclusion: The intensity of CD44v6 reaction has important guiding significance in the occurrence of colorectal cancer, malignancy, predictive metastasis and objective estimation of patient prognosis.
