目的探讨胃癌多药耐药(MDR)表达与预后,凋亡指数(AI)与预后及MDR与AI表达之间的关系。[方法]应用免疫 组化的方法及TUNEL法,结合1987年~1991年的完整胃癌病例进行10年随访,对胃癌MDR的主要指标P-gp、GST-π、DNA TOPO Ⅱ及细胞AI进行检测。[结果]P-gp表达与临床分期有关(P<0.05),正常组织表达P-gp高者生存期长(P<0.05)。GST-π表达强度 与预后有关(P<0.05)。TOPO Ⅱ表达与分型有关(P<0.05)。凋亡指数表达与临床分期有关(P<0.05)。P-gp里表达与GST-π表达 之间相关(P<0.05),GST-π与TOPO Ⅱ表达相关(P<0.05),但多药耐药表达与细胞AI之间无关(P>0.05)。[结论]MDR与AI都 属细胞正常基因组表达的一部分,MDR主要功能是细胞防御,其表达产物之间有相关性。细胞凋亡功能为细胞自稳,也由基因控 制,但与MDR调控基因无相关,不同生存年段其MDR与AI表达未见有明显统计差异(P>0.05)。提示单因素分析MDR/AI与预 后尚不够全面。
Objective To investigate the relationship between multidrug resistance (MDR) and prognosis, apoptosis index (AI), prognosis and MDR and AI expression in gastric cancer. [Methods] The immunohistochemical methods and TUNEL method were used to detect the expression of P-gp, GST-π, TOPO Ⅱ and AI in gastric cancer with 10-year follow-up of complete gastric cancer from 1987 to 1991 . [Results] The expression of P-gp was correlated with clinical stage (P <0.05), and the survival of P-gp in normal tissues was longer (P <0.05). The expression of GST-π was correlated with prognosis (P <0.05). TOPO Ⅱ expression was correlated with typing (P <0.05). The expression of apoptosis index correlated with clinical stage (P <0.05). The expression of GST-π in P-gp was correlated with the expression of GST-π (P <0.05), GST-π was correlated with the expression of TOPO Ⅱ (P <0.05), but there was no correlation between multidrug resistance and AI P> 0.05). [Conclusion] Both MDR and AI are part of normal cell genome expression. The main function of MDR is cell defense, and the expression products are correlated with each other. The function of apoptosis was cell self-stabilization and gene control, but no correlation with MDR regulated genes. There was no significant difference in MDR and AI expression in different survival stages (P> 0.05). Prompted univariate analysis of MDR / AI and prognosis is not comprehensive enough.
