目的分析漳州市新生儿先天性甲状腺功能减低症(CH)的筛查情况,总结地域因素和季节变化对新生儿促甲状腺素(TSH)筛查切值的影响。方法选取2013年3月-2015年2月新生儿138 760例,采用时间分辨荧光分析法测定新生儿足跟血的TSH浓度。结果城区的切值最高,为10.33μU/m L,沿海的切值为8.95μU/m L,山区的切值为8.15μU/m L;不同季节TSH切值不同,冬季明显高于其他3季,冬季切值达10.66μU/m L,春季8.88μU/m L,夏季8.49μU/m L,秋季8.98μU/m L。结论 TSH筛查切值存在季节和地域的差异,建议根据本实验室的具体情况制定TSH切值。
Objective To analyze the screening of congenital hypothyroidism (CH) in neonates in Zhangzhou City and summarize the influence of regional and seasonal changes on TSH screening value in neonates. Methods 138 760 newborn infants from March 2013 to February 2015 were selected. TSH concentration in neonatal heel blood was measured by time-resolved fluorescence analysis. Results The highest cut-value was 10.33μU / m L in urban area, the cut-off value was 8.95μU / m L in coastal area and 8.15μU / m L in mountainous area. The values of TSH cuttings in different seasons were significantly higher in winter than in other three seasons , Winter cut-off value of 10.66μU / m L, 8.88μU / m L in spring, 8.49μU / m L in summer and 8.98μU / m L in autumn. Conclusion There are seasonal and regional differences in TSH screening cut-off values. It is suggested that TSH cut-off values should be established according to the specific conditions in our laboratory.
