目的探讨凝血检验危急值在重症新生儿病房中的应用及其价值。方法选取2014年1月-2015年12月该院收治的300例重症新生儿作为研究对象,对其凝血检验结果与凝血功能进行回顾性分析,了解其临床应用价值。结果 300例重症新生儿中凝血检验危急值患儿32例(10.7%)。凝血检验危急值类型分布为Fg低值、APTT高值、PT高值、Fg低值合并APTT高值、Fg低值合并PT高值、APTT高值合并PT高值,构成比分别为53.1%、15.5%、12.5%、6.3%、6.3%、6.3%。32例凝血检验危急值患儿中Fg低值主要以新生儿重症肺炎、新生儿呼吸窘迫、新生儿脓毒血症、新生儿胎粪吸入综合征、高胆红素血症疾病为主,构成比分别为47.1%、17.6%、17.6%、11.8%和5.9%;APTT高值主要以新生儿脓毒血症、新生儿窒息为主,构成比为80.0%、20.0%;PT高值主要以新生儿呼吸窘迫、维生素K缺乏症为主,构成比为75.0%、25.0%;Fg低值合并APTT高值、Fg低值合并PT高值和APTT高值合并PT高值中分别以新生儿窒息、新生儿脓毒血症和新生儿呼吸衰竭为主,构成比均为100.0%。结论重症新生儿病房中通过测定凝血检验危急值具有重要的应用效果,可以及时地了解患儿的病情变化,极大地降低死亡率,临床应用价值较高。
Objective To investigate the application of coagulation test critical value in severe neonatal ward and its value. Methods 300 severe neonates admitted from January 2014 to December 2015 in our hospital were selected as the research object. The coagulation test results and coagulation function were analyzed retrospectively to understand its clinical value. Results Thirty-two children (10.7%) with critical value of coagulation test in 300 severe newborns. The distribution of critical type of coagulation test was low value of Fg, high value of APTT, high value of PT, high value of APTT with low value of Fg, high value of PT with low value of Fg and high value of PT with high value of APTT, with the proportions of 53.1% 15.5%, 12.5%, 6.3%, 6.3%, 6.3%. 32 cases of coagulation test critical value of children with low Fg mainly in neonatal severe pneumonia, neonatal respiratory distress, neonatal sepsis, neonatal meconium aspiration syndrome, hyperbilirubinemia-based disease, constitute (47.1%, 17.6%, 17.6%, 11.8% and 5.9% respectively). The high value of APTT was mainly neonatal sepsis and neonatal asphyxia, with the proportions of 80.0% and 20.0% Neonatal respiratory distress, vitamin K deficiency-based, with a ratio of 75.0%, 25.0%; Fg low value combined with APTT high value, Fg low value combined with PT high value and APTT high value PT high value with neonatal asphyxia , Neonatal sepsis and neonatal respiratory failure, the constituent ratio was 100.0%. Conclusion Severe neonatal ward by measuring the critical value of coagulation test has an important application effect, can be timely understanding of changes in children with conditions, greatly reducing mortality, clinical application of higher value.
