目的观察益气活血汤辅助治疗冠心病心绞痛的临床疗效及对血脂、炎症因子水平的影响。方法选取80例首都医科大学良乡教学医院冠心病心绞痛患者,按随机数字表分为观察组和对照组,每组40例。对照组采用西医常规方法治疗,观察组在对照组的治疗基础上再口服益气活血汤治疗,连续服用30 d,治疗结束后进行心绞痛及心电图疗效评判,比较两组治疗前、治疗后观察心绞痛发作频率、持续时间、心绞痛积分变化情况,空腹抽血进行血脂指标[甘油三酯(TG)、总胆固醇(TC)以及低密度脂蛋白(LDL-C)]及炎症因子[血清白介素(IL)-6、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)]检测。结果观察组临床疗效及心电图疗效分别为90.00%和87.50%,均明显高于对照组,差异有统计学意义(χ~2=6.765、9.392,P<0.05)。两组治疗后心绞痛发作频率、持续时间、心绞痛积分均较治疗前明显下降(P<0.05),观察组治疗后心绞痛发作频率、持续时间、心绞痛积分明显低于对照组(P<0.05)。两组治疗后TG、TC、LDL-C、IL-6、hs-CRP、TNF-α均较治疗前明显下降(P<0.05),且观察组明显低于对照组(P<0.05)。结论益气活血汤辅助治疗冠心病心绞痛的临床疗效确切,能有效调节血脂水平及抑制局部炎症反应,从而改善痛患者症状,提高临床效果及心电图效果。
Objective To observe the clinical efficacy of Yiqi Huoxue decoction in the treatment of angina pectoris of coronary heart disease and its effect on serum lipids and inflammatory cytokines. Methods Eighty patients with coronary heart disease and angina pectoris from Liangxiang Teaching Hospital of Capital Medical University were selected and randomly divided into observation group and control group with 40 cases in each group. The control group was treated with routine western medicine. The observation group was treated with Yiqi Huoxue Decoction on the basis of the control group for 30 days. Angina pectoris and electrocardiogram curative effect were evaluated after the treatment. Before and after treatment, the angina pectoris (TG), total cholesterol (TC) and low density lipoprotein (LDL-C)] and inflammatory factors [serum interleukin (IL) -6, hs-CRP, TNF-α). Results The clinical curative effect and electrocardiogram curative effect in the observation group were 90.00% and 87.50%, respectively, which were significantly higher than those in the control group (χ ~ 2 = 6.765,9.392, P <0.05). The frequency of angina pectoris, duration and angina pectoris after treatment were significantly lower than those before treatment (P <0.05). The frequency, duration of angina pectoris, angina pectoris score in the observation group after treatment were significantly lower than those in the control group (P <0.05). TG, TC, LDL-C, IL-6, hs-CRP and TNF-αwere significantly lower than before treatment in both groups (P <0.05), and the observation group was significantly lower than the control group (P <0.05). Conclusions Yiqi Huoxue Decoction is an effective adjuvant therapy for coronary heart disease with angina pectoris. It can effectively regulate blood lipid level and suppress local inflammatory reaction, thus improving the symptoms of patients with pain and improving the clinical effect and electrocardiogram effect.
