目的加强私立医院临床用血临督、管理,确保临床用血安全和需要。方法按卫生部《医疗机构临床用血管理办法》、《临床输血技术规范》要求对辖区内进行过输血治疗和抢救的私立医院进行现状调查。结果红河州开展和实施输血救治的私立医院共12所,占私立医院(诊所)的2.8%,私立医院用血量小,平均为200~1200ml/次,用血频率低,平均为0.65次/月,以急诊用血为主占83%,供血半径平均45.5km,送血时间平均为1h;12所私立医院均无血库和血液及制品保存设施,无相关的专业人员,血型签定、交叉配血不规范,输血环境差,无相关的输血操作规程和管理制度,无《输血同意书》、《输血申请书》,输血采取现要、现送、现输,血型、血量送错达9次占9.6%,因输血引发的医疗事故和纠纷多达12起,成份输血率仅为28.6%。结论私立医院临床用血不规范,必须加强监督和管理,建议:加强私立医院实施输血救治的条件、资质认定和考核;加强私立医院血库设施、人员的培训和建设,完善各项措施和操作规程、制度,加强边远地区血库网络建设,以确保输血治疗的安全和需要。
Objective To strengthen the supervision and management of clinical blood use in private hospitals to ensure blood safety and needs for clinical use. Methods According to the “Medical Measures for Clinical Use of Blood in Medical Institutions” and the “Technical Code for Clinical Blood Transfusion” issued by the Ministry of Health, a survey was conducted on the status of private hospitals that had undergone blood transfusion therapy and rescue in their jurisdictions. Results A total of 12 private hospitals in Honghe Prefecture carried out and implemented blood transfusion rescue, accounting for 2.8% of private hospitals (clinics). Private hospitals used a small amount of blood, averaging 200 to 1200 ml/time, with a low frequency of blood use, averaging 0.65 times/ In the month, 83% of the patients used emergency medicine, the average blood supply radius was 45.5km, and the average blood donation time was 1h; 12 private hospitals had no blood bank and blood and product storage facilities, no relevant professionals, blood type sign, cross Blood allocation is not standardized, blood transfusion environment is poor, there is no related blood transfusion procedure and management system, no “transfusion consent”, “blood transfusion application”, blood transfusion takes immediate, present delivery, is lost, blood type, blood volume delivery error 9 times accounted for 9.6%, as a result of blood transfusion-induced medical accidents and disputes up to 12 cases, component transfusion rate was only 28.6%. Conclusion The non-standard blood for clinical use in private hospitals must be strengthened in supervision and management. It is recommended that: strengthen the conditions, qualifications and assessment of blood transfusions in private hospitals; strengthen the training and construction of blood bank facilities and personnel in private hospitals, and improve the measures and procedures. And strengthen the construction of blood bank networks in remote areas to ensure the safety and needs of blood transfusions.
