目的 :运用磁共振弥散加权成像技术 (MRDWI)评估近似弥散系数 (ADC)在星形细胞瘤的细胞密度和病理分级中的作用。方法 :术前进行MRI和MRDWI检查且经手术病理证实的 3 4例星形细胞瘤患者 ,其中Ⅰ~Ⅱ级 (低级别 ) 2 6例 ,Ⅲ~Ⅳ级 (高级别 ) 8例。在MRI上计算瘤体的ADC值 ,运用AdobePhotoshop 7.0软件分析星形细胞瘤的细胞密度 ,用SPSS 10 .0软件对资料进行处理。结果 :高级别星形细胞瘤的ADC为 ( 7.3 4± 2 .95 )× 10 -4mm2 /s ,低级别星形细胞瘤的ADC ( 13 76± 3 3 1)× 10 -4mm2 /s ,两者之间差异有显著性 (t =4 913 ,P <0 .0 0 1) ;高级别星形细胞瘤的细胞密度为 ( 19 81± 9 73 ) % ,明显大于低级别星形细胞瘤的细胞密度( 4 74± 2 96) % (t =4 3 2 ,P <0 .0 0 3 ) ;星形细胞瘤的ADC值与细胞密度呈显著负相关 (r =-0 5 3 5 ,P =0 .0 0 1)。结论 :星形细胸瘤的细胞密度对磁共振弥散加权成像中的ADC值贡献很大 ;MRDWI对星形细胞瘤的细胞密度的预测和评价很有价值 ;MRDWI磁共振散加权成像具有对星形细胞瘤的良、恶性程度进行预测的潜能
OBJECTIVE: To evaluate the role of approximate diffusion coefficient (ADC) in the cell density and pathological grade of astrocytomas using magnetic resonance diffusion-weighted imaging (MRDWI). Methods: Thirty-four patients with astrocytoma who underwent MRI and MRDWI before operation and confirmed by surgery and pathology were enrolled in this study. There were 26 patients with grade Ⅰ ~ Ⅱ (low grade) and 8 patients with grade Ⅲ ~ Ⅳ (high grade). The ADC value of the tumor was calculated on MRI, the cell density of astrocytoma was analyzed by using Adobe Photoshop 7.0 software, and the data was processed by SPSS10.0 software. Results: The ADC of high-grade astrocytoma was (7.34 ± 2.95) × 10-4mm2 / s and that of low-grade astrocytoma (13 76 ± 3 3 1) × 10-4mm2 / s, (T = 4913, P <0.001). The cell density of high grade astrocytoma was (19 81 ± 9 73)%, which was significantly higher than that of low grade astrocytoma Cell density (4 74 ± 2 96)% (t = 4 3 2, P <0 0 3 3). The ADC value of astrocytomas was negatively correlated with cell density (r = -0.535, P = 0 .0 0 1). CONCLUSIONS: The cell density of astrocytomas has a great contribution to the ADC value in MR diffusion weighted imaging. MRDWI is valuable in the prediction and evaluation of cell density of astrocytomas. The prognostic potential of somatic and malignant tumors
