目的探讨阿尔茨海默病和多发梗死性痴呆患者CT检查改变与临床智能精神检查的相关性。方法选取近2年某院收治的37例阿尔茨海默病患者和35例多发梗死性痴呆患者为研究对象,探讨两种疾病CT影像学改变与临床智能精神检查得分的相关性。结果①阿尔茨海默病组前角、后角、基底池、外侧裂、三脑室、四脑室、侧脑室以及脑沟等部位的最大宽度值均大于多发梗死性痴呆组,比较差异有统计学意义(P﹤0.05)。②多发梗死性痴呆组MMSE评分为16.79±5.48,DRS评分为0.832,均高于阿尔茨海默病组(13.24±2.38,0.673),比较差异有统计学意义(P﹤0.05)。③阿尔茨海默病组MMSE评分与外侧裂和侧脑室宽度呈负相关;DRS评分与前角和四脑室宽度呈正相关(P﹤0.05)。④多发梗死性痴呆组MMSE评分与DRS评分与脑室系统无明显相关性(P﹥0.05)。结论 CT检查脑室系统影像学改变是鉴别阿尔茨海默病与多发梗死性痴呆的主要方面,脑室系统病理性改变与临床智能精神改变有直接关系。
Objective To investigate the correlation between the changes of CT in Alzheimer’s disease and patients with multi-infarct dementia and clinical mental examination. Methods A total of 37 patients with Alzheimer’s disease and 35 patients with multi-infarct dementia admitted to a hospital in recent 2 years were enrolled in this study. The correlation between CT findings and clinical intelligence scores of two diseases was explored. Results ① The maximum width of anterior horn, posterior horn, basal cistern, lateral fissure, third ventricle, fourth ventricle, lateral ventricle and sulcus in Alzheimer’s disease group were all greater than those in the group of multiple infarct dementia. There were statistically significant differences Significance (P <0.05). ② The MMSE score of multi-infarct dementia group was 16.79 ± 5.48, and the DRS score was 0.832, which was higher than that of Alzheimer’s disease group (13.24 ± 2.38, 0.673). The difference was statistically significant (P <0.05). ③ MMSE score of Alzheimer’s disease group was negatively correlated with lateral cleft and lateral ventricle width; DRS score was positively correlated with anterior horn and fourth ventricle width (P <0.05). ④ There was no significant correlation between MMSE score, DRS score and ventricular system in patients with multi-infarct dementia (P> 0.05). Conclusion CT examination of the ventricular system imaging changes is to identify the main aspects of Alzheimer’s disease and multiple infarction dementia, pathological changes in ventricular system and clinical intelligence is directly related to mental changes.
