目的:评价临床药师应用智能化用药安全警示互动系统(简称互动系统)在预防药物不良事件(ADEs)等方面的干预效果。方法:以肾内科为研究科室,运行互动系统,收集ADEs,统计警示信息,采用配对方法研究ADEs对住院费用及时间的影响;采用前后对照方法研究干预前后ADEs发生率变化情况。结果:应用互动系统之后,警示信息数显著下降,药师与医师互动率达35.3%,医师采纳率达80%;比较干预前组(2008年8月~2009年7月)和干预后组(2010年12月~2011年9月),标准化率得可预防ADEs发生率从8.2%下降至0.8%,预计每1000人可减少74例可预防ADEs,从而减少共约1 057 016元费用和999天住院时间。结论:互动系统作为一种新的干预手段,不仅有利于临床用药安全,降低可预防ADEs,也有利于发挥与培养临床药师的职责。
Objective: To evaluate the intervention effect of clinical pharmacists in the application of intelligent drug safety warning interaction system (referred to as interactive system) in the prevention of adverse drug events (ADEs). Methods: With the department of nephrology as the research department, an interactive system was run, ADEs were collected, statistical warning information was collected, and the effect of ADEs on hospitalization costs and time was studied by paired methods. The changes of ADEs before and after intervention were studied by using the pre-and post-intervention method. Results: The number of warning messages dropped significantly after the interactive system was applied. The interaction rate between pharmacists and physicians reached 35.3% and that of physicians reached 80%. Compared with the intervention group (August 2008 ~ July 2009) and the intervention group (2010) December 2011 ~ September 2011), the standardization rate of prophylaxis of ADEs decreased from 8.2% to 0.8%, and it is estimated that 74 cases of preventable ADEs can be reduced per 1000 people, resulting in a reduction of about 1 057 016 yuan and 999 days Hospitalization time. Conclusion: As a new intervention, interactive system is not only beneficial to the safety of clinical medication but also to the prevention of ADEs. It is also beneficial to play the role of clinical pharmacist.
