张某,男,65岁。1980年12月5日入院,住院号:51481。患者因高血压病入院,经中、西药治疗后,血压已下降至正常范围。12月12日,某医生见患者舌苔黄腻,且伴有口苦、纳差等症状,认为其内有湿热,即予清实热之剂牛黄解毒片4片口服,以冀通腑泻热,但药后翌日,患者呃逆顿起,并觉腹胀,拉稀便3次,夜间呃逆频繁而不能入眠。诊见:呃声低沉,神情倦怠,腹部平软无压痛,舌质淡红,舌苔淡黄,脉弦略滑。笔者考虑此呃逆为痰热挟胃气上逆所致,即
