[目的]了解某院2005~2008年产超广谱β内酰胺酶大肠埃希菌的耐药变化情况。[方法]对临床分离的大肠埃希菌采用K-B法进行药敏试验,用WHONET5.4软件进行数据统计分析。[结果]4年来大肠埃希菌产ES-BLs阳性率从45.5%上升至59.1%。产ESBLs株对头孢菌素类、氟喹诺酮类等常用抗菌药物的耐药率明显高于非产ES-BLs株,对碳青霉烯类抗菌药物敏感。[结论]4年来,大肠埃希菌中ESBLs阳性率逐年升高,并呈现出多重耐药性,对产ESBLs菌株的治疗应首选碳青霉烯类抗菌药物。
[Objective] To understand the resistance change of extended-spectrum β-lactamase-producing Escherichia coli from 2005 to 2008 in a hospital. [Method] The clinical isolates of Escherichia coli were tested for susceptibility by K-B method. Data were analyzed by WHONET5.4 software. [Results] The positive rate of Escherichia coli producing ES-BLs in 4 years increased from 45.5% to 59.1%. ESBLs-producing strains of cephalosporins, fluoroquinolones and other commonly used antimicrobial resistance rates were significantly higher than non-ES-BLs strain, susceptible to carbapenem antibacterial drugs. [Conclusion] The positive rate of ESBLs in Escherichia coli increased year by year and showed multi-drug resistance in 4 years. Carbapenem-based antimicrobial agents should be the first choice for the treatment of ESBLs-producing strains.
